Your Credentialing Roadmap

Each broker has its own application. Here's what you'll need for every submission — and what to expect from each.

Step 01

Get Your NPI Number

A National Provider Identifier is required for all billing. Without it, no broker will credential you.

  • Apply free at npires.cms.hhs.gov
  • Choose Type 1: Individual NPI
  • Select "Health Care Provider" taxonomy
  • Processing time: 1–5 business days
Step 02

Register Your Business

Your business must be a legal entity before brokers will contract with you.

  • Form an LLC or equivalent in your state
  • Get a federal EIN from the IRS (free)
  • Register with your state's Medicaid agency
  • Some states require a transportation permit
Step 03

Gather Vehicle Documents

Brokers require documentation for every vehicle you'll dispatch.

  • Vehicle registration (all vehicles)
  • Current state inspection stickers
  • Ambulance/PTV certification (if applicable)
  • ODometer reading at credentialing time
Step 04

Secure Commercial Insurance

NEMT operators carry higher insurance minimums than standard rideshare.

  • General liability: $1M per occurrence minimum
  • Commercial auto: $1M combined single limit
  • Workers' comp if you have W-2 drivers
  • Non-transporting ambulance coverage if billing as ambulance
Step 05

Pass a Background Check

Every driver on your roster needs cleared before you can be credentialed.

  • Fingerprint-based criminal background check
  • DMV driving record (no more than 3 points)
  • Sex offender registry clearance
  • Drug screening (panel 10 or greater)
Step 06

Submit Broker Applications

With everything in hand, apply to each broker in the states where you want to operate.

  • ModivCare portal: modivcare.com/provider-credentialing
  • MTM portal: mtmonline.net/transportation-providers
  • Processing: 30–90 days per broker per state
  • Some states require a separate state Medicaid enrollment

What Every Broker Requires

No matter which broker you're applying to — these don't change.

🪪 NPI Number

Individual Type 1 NPI. Free to apply at npires.cms.hhs.gov. Takes 1–5 days.

🏢 Business Registration

LLC or equivalent. Federal EIN. State Medicaid provider enrollment where required.

🚐 Vehicle Documents

Registration, inspection certificates, ambulance/PTV certs if applicable. All vehicles listed on contract.

🛡️ Commercial Insurance

$1M GL + $1M commercial auto minimum. Named insured must match broker contract.

🔍 Background Checks

Fingerprints, DMV records, sex offender registry, drug screening for all drivers.

📋 W-9 Form

Used for 1099 contractor setup. Have it ready — you'll need it for every application.
